Mental Health Stepped Care "LIFT" - West Heidelberg
LIFT stepped care model for mental health provides expert support from mental health nurses, psychologists, counsellors, care coordinators, and peer workers to build on strengths and support an individual's recovery from mental illnesses including anxiety, depression, chronic mental illnesses, and suicidal thoughts.
LIFT stepped care model ensures that every person seeking help with a mental health concern receives the right care at the right time from the right person. The program is flexible to meet the individual's changing needs and recognises that the impact of their mental health concerns will vary over time.
LIFT provides lower-level to higher-intensity supports such as:
- Online support
- Counselling (face-to-face or via telehealth)
- Group support
- Care coordination
- Referral to other services to address other needs, including problem gambling, substance misuse, family violence, employment, education, training, and financial support services
- General practitioner to support overall physical health and wellbeing
Eligibility Criteria
The service is for people of all ages who:
- Live, work or study in the Cities of Banyule and Whittlesea, Shire of Nillumbik, and the surrounding areas around Wallan and Kinglake
- Otherwise could not afford similar services
