How SupportConnect works
Whether you are looking for support for yourself, you're a family member, carer or friend, or a professional, SupportConnect is here to help you find the right mental health, alcohol and other drugs and suicide prevention services in eastern and north-eastern Melbourne.
You can search for the services that best meet your needs by answering a few simple questions, or you can search by typing what you’re looking for into the search bar. You can also call a member of the SupportConnect team if you’d prefer to talk to a person.
SupportConnect also has a resource section. This section includes a number of links to trusted websites with information on a variety of topics on mental health, social and emotional wellbeing, suicide prevention, and drugs, alcohol and addictive behaviour.
What is the Eastern Melbourne Primary Health Network
SupportConnect has been developed by the Eastern Melbourne Primary Health Network (EMPHN), to support people who live, work or study in eastern and north-eastern Melbourne to find the right mental health, alcohol and other drugs and suicide prevention services.
The Australian Government established Primary Health Networks (PHNs) to increase the efficiency of health services, reduce fragmentation of care and improve health outcomes for everyone, especially for the most vulnerable. There are 31 Primary Health Networks in Australia and six in Victoria.
EMPHN achieves this goal by improving access to existing services, funding services to improve health outcomes and by supporting GPs and others to further improve local health care. EMPHN aims to improve the health of the community by ensuring people receive the right care, in the right place, at the right time.
