Conduit Health

• Conduit Health aims to deliver specialist health care in an effective, convenient and readily accessible way for the general population, whilst providing additional support for primary care physicians.
• Combining the availability of information technology and professional health care, they are able to provide access to specialist services, especially to those living in rural areas.
• Conduit Health consults, there is no travel time, and the consults can happen in the comfort of one’s home.
• Conduit Health also assists General Practitioners to get a shared health care plan by a specialist for their patients, in a timely fashion. In some cases, they are able to provide access between the General Practitioner and Psychiatrist to discuss the patient’s care via teleconference.

People living rural or remote, or on outer fringes of Melbourne (eg Whittlesea, Nillumbik, Yarra Ranges), are eligible.
